Letters of Recommendation
The Dolly Daisy Scholarship requires applicants to have two Letters of Recommendation submitted on their behalf. Follow the instructions below to complete and submit a Letter of Recommendation.
All Letters of Recommendation must be received by February 28, 2025.
Submission Instructions:
Download the Letter of Recommendation form. Both personal and academic letters can be written on the same form.
The form is a fillable PDF form. Please fill out the recommendation letter completely and save it.
Using Google Chrome or Adobe Reader is recommended.
Attach and email the completed form to:
Include the applicant's name in the subject line of the email.
If completing an academic letter: If possible, please submit the form using your school (.edu) email address.
